加巴喷丁治疗带状疱疹急性期神经痛的临床研究

目的探讨应用加巴喷丁（gabapentin）治疗带状疱疹急性期神经痛的临床效果与安全性。方法带状疱疹急性期患者32例，随机分为安慰剂组和加巴喷丁组，每组16例，均给常规药物治疗，加巴喷丁组在此基础上给予口服加巴喷丁，剂量自300mg／d起，根据服药后患者疼痛的缓解程度以及不良反应予以调整，每2d增加剂量1次，直至VAS≤3，然后以此剂量维持，疗程4周。分别于治疗前及治疗后1、2、4周采用视觉模拟评分（VAS）进行疼痛评估。结合24h疼痛持续时间和睡眠质量评分（QS）评定治疗效果。同时观察患者的不良反应。结果加巴喷丁组患者VAS评分、24h疼痛持续时间显著降低（P〈0．01），睡眠状况也明显改善（P〈0．01）。镇痛起效最低剂量为300mg，1次／d，80％患者每日有效镇痛剂量在300-900mg，日最大剂量达1200mg。结论带状疱疹急性期应用加巴喷丁可显著缓解疱疹疼痛症状。

Gabapentin for the treatment of neuropathic pain in acute phase of zoster

Objective To observe the effects and safety of gabapentin for oral use to treat acute phase of zoster. Methods Thirty two patients with zoster in acut ephase were divided ramdomly into placebo group and gabapentin group with 16 cases in each group. Both groups were treated with routine methods and gabapentin group was given gabapentin additionally. The beginning dose of gabapentin was 300 mg every day,Then adjusted according to the alleviating degree of pain and side effects. The dose was increased every two days till visual analogue scale （VAS） was under 3, then was maintained for four weeks. The pain scale was assessed at the beginning of the study and after 1 week,2 weeks and 4 weeks. The curative effect was evaluated by visual analogue scale （VAS）,24 h pain time and quality of sleep（QS）. Adverse reactions were also observed. Results There was significant reduction in VAS and 24 h pain time in gabapentin group （P〈0.01）, sleep quality was improved （P 〈0.01 ）. The minimal dosage of beginning analgesia was 300 mg once a day. The dosage of effective analgesia of 80 percent of patients was from 300 mg to 900 mg. The maximal dosage was 1 200 mg. Conclusion Use of gabapentin in acute phase of zoster may remit the pain symptom effectively.
