残胃功能性排空障碍的治疗

①目的:探讨胃大部切除术后残胃功能性排空障碍的病因、诊断及治疗措施。②方法:回顾性分析我院1988年8月～2002年12月发生的胃大部切除术后胃功能性排空障碍56例病人的临床资料。③结果:56例中44例行保守治疗，12例因误诊为输出端机械性梗阻而再次手术，全部治愈出院。④结论:残胃和空肠上段的正常运动功能障碍是发生本病的主要原因，与手术损伤胃和空肠上段的自主神经有关。非手术治疗是主要手段，应尽量避免早期再次手术。

TREATMENT OF FUNCTIONAL DELAYED GASTRIC EMPTYING AFTER SUBTOTAL GASTRECTOMY

Objective To determine the etiology, diagnosis and treatment of functional delayed gastric emptying(FDGE)following subtotal gastrectomy. Methods The clinical data of 56 FDGE patients after subtotal gastrectomy from 1988 to 2002 were analyzed retrospectively. Results All the 56 patients were recovered and discharged. Of which, 44 received conservative treatment, and 12 were misdiagnosed as having efferent mechanical obstruction and re-operated. Conclusion The main cause of FDGE is disturbance of gastrointestinal motility, which may be caused by vegetative nerve injury during the procedure. The main therapy is non-surgical. A re-operation should be avoided as possible at the early stage.
