CT灌注扫描在肺肿瘤的临床应用价值

目的研究肺部良恶性肿瘤多层螺旋CT(MSCT)各灌注参数的改变和良恶性肿瘤的鉴别诊断。方法 40例患者中良性病变组6例、恶性肿瘤(肺癌)组34例,均行肺血流的CT灌注扫描。应用去卷积算法模式计算相应病变区域的血流量(BF)、血容量(BV)、平均通过时间(MTT)、毛细血管通透性(PS)。根据不同肿瘤的相关参数图来评价良恶性肿瘤血流动力学状态。结果 34例肺癌患者BF、BV和PS明显高于良性病变,其中PS值>25mL·(100g·min)-1]的敏感性为97.06%、特异性为83.33%,而MTT值与良性病变无显著差异。结论灌注参数BF、BV和PS可有效的评价肺部病变的血流状态,在肺部良恶性病变的鉴别诊断上有重要的临床应用价值。

Application of clinic in lung tumor with MSCT perfusion imaging

【Objective】To study MSCT perfusion imaging of fine or malignancy tumor of lung and differentiate diagnose.【Methods】 dynamic CT with singe-location sequences CT scan of the lung were obtained in 40 individuals who include 6 patients with fine process and 34 patients with malignancy tumor(lung cancer).Lung perfusion parameters include blood flow(BF),blood volume(BV),mean transit time(MTT) and permeability surface(PS).【Results】 Compared with fine process of lung,BF,BV and PS in lung cancer group increased significantly,The PS>25 mL·(100g·min)-1]was set as a diagnostic threshold,the sensitivity and specificity were 97.06% and 83.33%,and MTT has not differ between two group.【Conclusion】 Lung perfusion parameters(BF,BV and PS) measured with CT were significantly altered in lung tumor,There have importance appliance value in differentiate diagnose of fine or malignancy tumor.
