摘    要:目的观察骨折延迟愈合患者血液流变学指标和血清细胞间粘附因子-1(sICAM-1)、血管细胞粘附分子-1(sVCAM-1)水平指标的变化及其意义。方法选取2007年1月至2011年12月于本院进行治疗的120例骨折患者作为研究对象,其中60例骨折延迟愈合患者为观察组,另外60例骨折适时愈合患者为对照组。采用酶联免疫法检测两组患者的sICAM-1及sVCAM-1水平,同时空腹抽取静脉血,检测患者血液黏稠度、血浆黏度、红细胞压积、纤维蛋白原及血沉,对上述指标进行比较分析。结果观察组患者的血清sICAM-1及sVCAM-1水平均高于对照组(P均<0.05),血液黏稠度、血浆黏度、红细胞压积、纤维蛋白原及血沉明显高于对照组(P均<0.05),差异均具有统计学意义。结论骨折愈合延迟会导致骨折患者处在慢性炎症反应状态,同时改变患者血液流变学,故动态检测患者血液流变学指标可作为诊断及了解病情的指标,预防骨折不良的发生。

Abstract:Objective To observe the changes in hemorheological indexes, serum intercellular adhesion molecule-1 (sICAM-1), vascular cellular adhesive molecular-1 (sVCAM-1) of patients with delayed union and its significance. Methods One hundred and twenty patients with fracture admitted into our hospital from January 2007 to December 2011 were selected as the research objects, including 60 patients with delayed union (the study group) and 60 patients with fracture healing timely (the control group). ELISA was applied to detect the levels of sICAM-1, sVCAM-1, and venous blood was extracted to detect the blood viscosity, plasma viscosity, hematocrit, fibrinogen and erythrocyte sedimentation rate (ESR). Results The levels of sICAM-1 and sVCAM-1 in the study group were significantly higher than those in the control group (P<0.05). The blood viscosity, plasma viscosity, hematocrit, fibrinogen and ESR in the study group were significantly higher than those in the control group (P<0.05). Conclusion Delayed union leads to chronic inflammatory response and changes in hemorheological indexes. Dynamic measurement of hemorheological indexes can be used to understand the disease, and prevent the happening of bad fractures.
