摘    要:目的研究不同分析前处理方法对比浊法凝血仪检测结果的影响。方法将乳糜血20份、健康人血20份,分别用常规法、高速离心法和手工法进行PT、APTT检测;将溶血标本21份、血量不足标本30份和重新补抽来的51份标本同时进行PT、APTT检测。结果健康人标本常规法、高速离心法、手工法检测的PT,APTT结果差异无统计学意义(P>0.05)。而乳糜血标本常规法检测PT、APTT结果高,甚至检测不出,经高速离心处理后结果与手工法比较差异无统计学意义(P>0.05);溶血、血量不足等问题标本的PT、APTT结果与重新采血后标本的结果比较,差异均有统计学意义(P<0.05),特别是血量不足时APTT明显延长。结论乳糜血、溶血、血量不足等问题标本大大影响了光电比浊法凝血仪检测结果的准确性,分析前标本的处理是保证其检测结果可靠性的一个重要因素。

Abstract:Objective To investigate the influence on test results with different pre-processing by applying turbidimetric method on blood coagulation analyzer. Methods We detected PT and APTT from 20 chylous blood specimens and 20 blood specimens of healthy individuals by routine method, high speed centrifugation and manual method, respectively. PT and APTT were also measured from 21 hemolytic specimens, 30 insufficient blood volume specimens and 51 re-sampling specimens at the same time. Results No statistically significant differences was found between PT and APTT results of the healthy individuals by routine method, high speed centrifugation and manual method (P>0.05). There was also no statistically significant difference between PT and APTT results of chylous blood specimens by high speed centrifugation and manual method (P>0.05), but the results of PT and APTT by routine method were higher. Differences in hemolytic specimens and insufficient blood volume specimens before and after re-sampling was statistically significant, and the results of APTT were significantly higher in insufficient blood volume specimens (P<0.05). Conclusion Problems such as chylemia, haemolysis and insufficient blood significantly influenced the accuracy of blood coagulation analyzer based on turbidimetric method. Pre-processing is an important element which ensures the reliability of test.
