| 摘 要： | 目的:研究全直肠系膜切除术(TME)在直肠癌治疗中的作用。方法:复习了国内外有关根治性直肠癌治疗中TME的概念和原则,并与传统手术进行了比较。结果:Heald总结了112例根治性切除术表明,5年内复发率为2.7%,5年生存率为87.5%。Enker报告了246例可治疗的Dukes B和C级病例中,局部复发仅18例(7.3%),5年生存率74.2%。但与传统手术相比,TME手术瘘的发生率较高。结论:选择TME术,使直肠癌手术后局部复发的控制和生存率明显改善。降低了预防性结肠造口、阳痿的发生等。提高了保肛率。

| Abstract: | Objective:To study the effect on TME in treating rectal.Methods:Reviewing the concepts and principles about TME in effect radical cancre the rectal cancer in relative document home and abroad,and compare with the tradition operations.Results:Heald summarized 112 example about radical cure,and showed that the relapse rate is 2.7% in five years,the survival rate is 87.5% in five years,Enter reported 246 cases in Dukes B and C that could be treated,only 18 cases relapse,the exist rate in five years in 74.2%.However compare with traditional operations,the operation fistula is higher.Conclusions:The TME technology makes the relapse controlled and it improves the exist rate in five years,and it avoid colostomy,asynodia and blood transfusion,etc.Unfavourable,the fistula is higher,because protecting the sphincter from injurying excessively. |
