复旦大学附属中山医院培训全科医师方法初探

日的 探讨在教学医院中全科医学科的住院医师培训方法。方法 对自1994年6月-2000年7月从上海医科大学五年制本科毕业后的20名住院医师，按全科住院医师的规范化培训要求，时间3-5掉，考核并追踪他们在内科，全科病区，急诊及社区卫生服务站点工作的能力，并与同期毕业的内科医师作比较。结果 以全科执业医师为目标，经规范化的住院医师培训，五年后均能顺利地晋升为全科主治医师，他们能胜任一般内科或其他专科医师不能承担的工作，工作范畴之广涉及到围生期的保健，婴幼儿的预防接种，计划生育技术指导，慢性病的防治，内、外、妇、儿、五官、眼科等疾病的诊治。心理咨询，残疾后的康复乃至临终关怀，并在本地卫生局举办的全科医师岗位培训班及各类医师继续教育中担任师资，撰写论文、科普文章20余篇，结论 本培训方法尚有成效，但存在师资缺乏，生源困难及教育经费不足的问题。

The primary study of methods in general practitioners' training

Objective To study the training methods in general practice in teaching hospital.Methods During six years from June,1994 to July,2000,according to the regular training requirements for general practitioners,twenty residents who graduated from Shanghai Medical University were tested for the qualifications of their work in internal medicine,general practice ward,emergency department and community health service,and were compared with the physicians homograduated.Results The residents will become general practitioners successfully after five-year regular training. They are competent for the diverse tasks that the physicians and the other specialists cannot take on,including perinatal care,infants and children vaccination,planned parenthood,prevention and therapy of chronic diseases,diagnosis and treatment of common diseases of internal medicine,surgery,gynecology,pediatrics,otolaryngology and ophthalmology,psychological counseling,rehabilitation and hospice care.These general practitioners are also the teachers in both the on-the-job training of general practitioners and the continuing medical education.Conclusion The training method is effective,but there still exist some problems,including lack of teachers,the source of students and educational outlays.
