| 摘 要： | 目的:探讨老年COPD患者并自发性气胸的原因、诊断、治疗及影响预后的相关因素。方法:对41例住院的老年COPD并自发性气胸患者的临床资料进行回顾性分析,并和30例非老年COPD并自发性气胸组对照。结果:老年COPD并自发性气胸的相关因素有:年龄,基础疾病与伴发症,负重或咳嗽,治疗和抢救不当等。结论:老年COPD并自发性气胸起病危急且不典型,并存病症多,恢复慢,易复发,易误诊,病死率高。由于单纯抽气治疗不易奏效,宜尽早作胸腔闭式引流排气。

| Abstract: | Objective: To explore the reason, diagnosis and treament methods of geriatric chronic obstructive pulmonary disease (COPD)complicated by spontaneous pneumothorax and the related factors affecting its prognosis. Methods: The clinical date of 41 cases of geriatric COPD complicated by spontaneus pneumotherax were retrospectevely analysed and compared with that of 30 cases of spontaneous pneumothorax caused by nongeriatric COPD. Results: The related factors affecting the geriatric COPD complicated by spontaneous pneumothorax are the age,basal diseases and complications carrying heavy load or cough improper treament and rescue, etc.Conclusion:The geriatric COPD complicated by spontaneous pneumothorax erupts criticaly,is accompanied by many complications, recovers slowly and recurs easily.This kind of disease is easy to be wrongly diagnised and has a high death rate. |
