适形放疗与加近距离放疗治疗食管癌疗效对比研究

目的：探讨适形放疗加近距离治疗,能否提高食管癌的疗效。方法：2004年1月～2006年12月60例食管癌患者,随机分为研究组和对照组。对照组只做适形放疗,总剂量60～70 Gy/6～7周,研究组适形放疗60 Gy后改用高剂量率192Ir腔内治疗,总剂量70 Gy。结果：对照组与研究组的近期有效率、1年局控率和生存率相似,2、3年局控率和生存率研究组高于对照组。研究组放射性食管炎发生率较高,放射性肺炎的发生率相似。结论：采用适形放疗加近距离放疗能提高食管癌的2、3年局控率和生存率。与适形放疗相比未明显增加放射性食管炎及放射性肺炎的发生率。

A Comparative Study on the Clinical Effect of Conformal Radiotherapy(CRT) and CRT Plus Brachytherapy in the Treatment of Patients with Esophageal Cancer

Objective：To explore the radiotherapy mode of the esophageal cancer so as to enhance the local control rate and improve the survival rate but not to increase radiation complications.Methods： Sixty inoperable esophageal cancers for various reasons between January 2004 to December 2006 were treated by CRT plus the intracavitary brachytherapy（IBT）（study group） and CRT（control group）.The total dose of the control group is 60～70 Gy/6-7 w and the study group were treated with HDR-192Ir IBT after 60 Gy to 70 Gy total dose.Results： The recent respective rates and 1-year local control rate and 1 year survival rate of the control group and the study group were similar.The 2 year and 3 year local control rates and survival rates were of difference.The esophagitis and pneumonia were not statistically significant.Conclusion： CRT plus IBT can improve 2 and 3-year local control rate and survival rate.Compared to the esophageal cancer with CRT,it does not increase the incidence of esophagitis and pneumonia.
