老年人社区获得性肺炎的病原菌检查及耐药性分析

摘    要:目的探讨社区获得性肺炎(CAP)细菌病原构成及耐药性的差异,为临床提供参考。方法对本院2011年3月~2012年2月收治的CAP老年患者235例进行痰细菌学培养及致病菌耐药性分析。结果 235例CAP患者共培养细菌189株,细菌构成前5位分别为肺炎链球菌、肺炎克雷伯菌、铜绿假单胞菌、大肠埃希菌、鲍曼不动杆菌。对氨苄西林+舒巴坦、四环素、氨曲南、复方新诺明、头孢他啶耐药率明显高于其他抗生素。结论 CAP感染病原菌的分布与耐药性都在不断发生变化,给临床治疗带来了很大困难,为了防止耐药菌株的产生,要求临床医生中工作中运用合理的抗菌药物应用策略,并对病原体的分布和耐药性进行动态监测。

Abstract:Objective To investigate the differences of bacterial and pathogens constitution and drug resistance of community acquired pneumonia(CAP),so as to provide reference for clinic.Methods Two hundred and thirty five cases of elderly patients with CAP admitted to our hospital from March 2011 to February 2012 were performed bacterial culture of sputum and drug resistance analysis of pathogenic bacterium.Results There were 189 strains cultured in the 235 patients with CAP.The top five bacterial constitutions were streptococcus,klebsiella,pseudomonas,escherichia coli,Bauman acineto bacter respectively.The drug resistance rates of ampicillin plus sulbactam,tetracycline,aztreonam,cotrimoxazole,cef tazidime were significantly higher than other antibiotics.Conclusion The constitution of pathogens and drug resistance of CAP infection are changing constantly,which has brought great difficulties for clinical treatment.In order to prevent the development of resistant strains,it is required that the clinician should use reasonable strategy of antibacterial drugs and conduct dynamic monitoring of the pathogen distribution and drug resistance.
