剖宫产率及指征变化对围产儿死亡率的影响

目的探讨剖宫产率及剖宫产指征变化对围产儿死亡率的影响。方法对1994年1月至2003年12月间我院2434例剖宫产病例资料进行回顾性分析。结果(1)剖宫产率呈逐年上升趋势;(2)在剖宫产指征中,难产因素和胎儿窘迫持续在1、2位,社会因素逐年上升;(3)围产儿死亡率无显著性差异(P>0.05)。结论剖宫产率升高在一定范围内降低了围产儿死亡率,但剖宫产率的进一步升高,围产儿死亡率并未随之下降。因此,应合理掌握剖宫产指征,降低剖宫产率。

The effects of cesarean section rate and changes of sesarean section indication on the perinatal mortality rate

Objective To study the effects of cesarean section rate and changes of cesarean section indication on the perinatal mortality reat. Methods 2434 cesarean section cases occurred in our hospital form Jan 1994 to Dec 2003 were analyzed retrospectively. Results (1) The rate had increased gradually year after year; (2) In the cesarean section indications, dystocia and fetal distress was always the first or second, social factor rises was increased gradually year after year; (3) There was no significant difference in the perinatal morality rate (P<0.05). Conclusions Within certain limits, the rise in cesarean section rate can reduce perinatal mortality rate. But with the further rise of cesarean section rate, the perinatal mortality rate dose not decrease. Therefore, we should have a grasp of cesarean section indication to decrease cesarean section rate.
