摘    要:目的 探讨CD46 rs1970530变异与肺腺癌易感性和预后的关系。方法 该病例对照研究包括400例肺腺癌患者和600例正常对照。使用χ2检验比较病例组和对照组中性别、年龄和吸烟状况之间的分布差异。使用PCR -限制性片段长度多态性分析对CD46 rs1970530遗传变异进行基因分型。使用非条件logistic回归分析CD46 rs1970530变异与肺腺癌易感性的关系。使用Kaplan - Meier生存曲线、Log - Rank检验和Cox多因素回归模型评估CD46 rs1970530变异各基因型患者间的预后差异。通过UALCAN数据库分析CD46基因在肺腺癌组织及其癌旁正常组织中的表达差异,使用Kaplan - Meier Plotter数据库分析CD46表达对肺腺癌患者总体生存时间的影响。结果 CD46基因在肺腺癌组织中的表达显著高于正常组织,且CD46高表达肺腺癌患者的总体生存时间显著高于CD46低表达患者。与CD46 rs1970530 AA基因型携带者相比,至少携带一个G等位基因的肺腺癌患者的发病风险显著增加(OR = 1.52,95%CI:1.10~2.09)。不同性别和年龄的中位生存时间不存在统计学差异(P>0.05)。在肺腺癌患者中,不吸烟者的中位生存时间(48个月)显著高于吸烟者(19个月)(P = 0.001)。携带CD46 rs1970530 GG基因型的肺腺癌患者的中位生存时间(10个月)显著低于AA基因型(35个月)和AG基因型患者(56个月)(P = 0.002)。多因素COX回归分析显示,CD46 rs1970530GG基因型是肺腺癌患者预后的高危因素(HR = 5.15,95%CI:1.81~14.66)。结论 CD46 rs1970530变异可以增加肺腺癌的发病风险,并提示预后不良。

Abstract:Objective To investigate the association of CD46 rs1970530 variant with the risk and prognosis of lung adenocarcinoma. Methods A total of 400 patients with lung adenocarcinoma and 600 normal controls were involved in this study. The χ2 test was used to compare differences in the distribution of sex, age, and smoking status between the case and control groups. For CD46 rs1970530 polymorphism, all samples were genotyped by PCR-restriction fragment length polymorphism. Unconditional logistic regression model was used to analyze the relationship between CD46 rs1970530 variation and the susceptibility to lung adenocarcinoma. Kaplan-Meier survival curve, Log-Rank test and Cox multivariate regression model were used to evaluate the prognostic differences among patients with CD46 rs1970530 variant. UALCAN were used to analyze the expression of CD46 gene in lung adenocarcinoma and adjacent normal tissues. Kaplan-Meier Plotter were used to analyze the effect of CD46 expression on the overall survival of lung adenocarcinoma patients. Results The results showed that the expression of CD46 in lung adenocarcinoma tissues was higher than that in normal tissues. The overall survival time of lung adenocarcinoma patients with high CD46 expression was longer than that of patients with low CD46. Compared with AA genotype carriers, those with at least one G allele had increased risk of lung adenocarcinoma (OR=1.52, 95%CI: 1.10-2.09). Survival analysis didn’t show any significant difference in median survival time between different gender or age groups (P>0.05). The median survival time of non-smoking patients was 48 months, which was significantly higher than that of smokers (P=0.001). The median survival time of patients with CD46 rs1970530GG genotype was 10 months, which was significantly lower than that of carriers with AA genotype (35 months) or AG genotype (56 months) (P=0.002). CD46 rs1970530GG genotype was a high-risk factor for prognosis in patients with lung adenocarcinoma (HR=5.15, 95%CI: 1.81-14.66). Conclusion CD46 rs1970530 variant increases the risk of lung adenocarcinoma and indicates a poor prognosis.
