新辅助动脉栓塞化疗在局部晚期宫颈癌治疗中的价值

目的：评价新辅助动脉栓塞化疗在局部晚期宫颈癌患者治疗中的作用。方法：将75例局部晚期宫颈癌患者随机分为两组，各例均行宫颈癌根治术，研究组35例于术前两周行髂内动脉栓塞化疗。结果：研究组患者化疗后肿瘤直径明显小于治疗前（P=0．000），化疗后肿瘤细胞的病理分级明显下降（P=0．004），肿瘤细胞的S期增高比率明显下降（P=0．001），凋亡峰明显增多（P=0．016），脉管浸润及淋巴结转移率明显低于对照组（P：0．007，P=0．036），2年复发率低于对照组（P=0．017）。结论：新辅助动脉栓塞化疗可有效缩小肿瘤，降低肿瘤细胞的恶性程度和肿瘤的脉管浸润和淋巴结转移率，诱导肿瘤细胞凋亡，降低S期增高比率，减少术后复发。

Effect of Neoadjuvant Arterial Chemoembolization on Locally Advanced Carcinoma of Cervix

Objective: To evaluate the effect of neoadjuvant arterial chemoemblization on locally advanced cervical carcinoma. Methods: 75 patients with locally advanced carcinoma of the cervix were randomly divided into two groups. All patients were received radical hysterectomy, 35 cases were received arterial chemoembolization two weeks before surgery. Results: The tumor diamonds of research group after chemotherapy were significantly smaller than those before chemotherapy (P = 0. 000). After chemotherapy the pathobiology grades displayed updown direction(P=0. 004), and the rates of heighten S period significantly declined(P=0. 001) ,the apoptosis peaks were clearly increased(P=0. 016). The rates of the vessel invasion and pelvic lymph node metastasis in in research group were significantly lower than those in comparisional group(P=0. 007,P = 0. 036). The 2-year recurrent rate in research group was lower than those in comparisional group(P=0. 017) Conclusion: The neoadjuvant arterial chemoemblization can effectly reduced tumor volume, decrease the pathobiology grades, ecrease the rates of the vessel invasion and pelvic lymph node metastasis , it can also induce the apoptosis of tumor cells , decrease the rates of heighten S period and the postoperative recurrence rate.
