胰腺血管的多层CT血管成像双期扫描技术初步研究

摘    要:目的:探讨多层螺旋CT胰腺血管成像的最佳扫描参数.方法:对40例纳入本研究的病人行多层螺旋CT双期血管成像研究.扫描时动脉期采用bolus-tracking技术;门脉期采用test-bolus技术.使用最大强度投影(MIP)和容积重建法(VR)重建胰腺动脉和静脉.结果:MIP、VR对胰腺间接供血动脉和门静脉、肠系膜上静脉、脾静脉、胃网膜右静脉的显示率均为100%,对胃结肠干均为80%.MIP、VR对胰十二指肠上后动脉的显示率分别为77.5%、67.5%;胰十二指肠上前动脉,82.5%、75%;胰十二指肠下动脉,67.5%、57.5%;右结肠上静脉,87.5%、80%;胰十二指肠上后静脉,65%、40%;胰十二指肠上前静脉,62.5%、45%.结论:使用多层螺旋CT,结合扫描前准备、适当的层厚、合理的延迟时间、一定的造影剂剂量和注射速率可获得良好的胰腺血管显示.

Abstract:Objective:To discuss the optimal scanning parameters of MDCTA of pancreas with multidector-row CT. Materials and Methods: Dual-phase CTA using a Siemens Sensation 16 scanner was performed in 40 cases meeting the including and excluding criteria. Acquisition of arterial and portal phase scan was obtained with bolus-tracking and test-bolus techniques, respectively. Pancreatic vessels were evaluated by axial images, MIP and VR. Results: The indirect pancreatic feeding arteries, PV, SMV, SV, RGEV were all seen in each technique. In MIP, VR, visualization rates of PSPDA were 91.5% and 67.5% respectively,ASPDA, 92.5% and 80%; IDPA, 67.5% and 57.5%; RSCV, 87.5% and 80%; PSPDV, 65% and 40%; ASPDV, 62.5% and 45%. Conclusion: Conspicuity of pancre-atic CTA can be gained using MDCT with combination of preparation prior to scanning, optimal slice thickness and scanning time, enough dosage and inject rate of contrast materials.
