神经生长因子与明胶海绵复合体修复骨缺损的实验研究

摘    要:目的:观察外源性神经生长因子(NerveGrowth Factor,NGF)与明胶海绵(GelatinSponge,GS)复合体(NGF/GS)对兔胫骨洞形骨缺损愈合中的作用。方法:18只健康成年新西兰大白兔的双侧胫骨分别制备2个5×3mm2深达髓腔的骨缺损,间隔10mm。其中实验组骨缺损区植入NGF/GS复合体(含NGF10μg),对照组植入GS,空白组不做任何处理。分别于术后2周、4周和8周各处死动物6只并获取标本,行标本大体、放射学及组织学观察,并进行骨计量学分析新生骨的面积百分比。结果:术后各组均未出现免疫反应。术后2、4周,各组新生骨密度低于正常骨,实验组与对照组和空白组相比,新生骨形成较快,骨缺损区密度较高,新生骨面积百分比明显增加。新生骨面积百分比:实验组2、4周(33.728±6.377)%、(55.854±6.842)%;对照组2、4周(22.139±4.346)%、(46.047±3.998)%;空白组2、4周(19.809±3.105)%、(39.250±6.151)%。术后8周,各组骨缺损基本愈合,骨密度接近。结论:神经生长因子与明胶海绵复合体能够促进兔胫骨洞形骨缺损的愈合,从而缩短骨损伤的愈合时间。

Abstract:Objective: To evaluate the effect of locally applied gelatin sponge incorporating nerve growth factor on bone regeneration in rabbit tibia defect.Methods: 18 new Zealand rabbits(weight 2-2.5kg) were randomly divided into three groups.Two bone defect(5mm long 3mm wide) were symmetrically created in both tibia under general anaesthesia,the distance between two defects was 10mm,and in total there were four bone defects for each rabbit.Then the gelatin sponge containing 10μg NGF solution were implanted into the defects as NGF/GS group,the gelatin sponge alone were implanted as GS control and the control group was left empty.6 rabbits were sacrificed at 2 weeks,4 weeks and 8 weeks respectively.Specimens were subjected to general observation,radiography,bone histological and finally histomorphometric analysis determining the new bone percentage,the data was analyzed with SPSS11.5 software package.Results: There was no sign of immune reaction to the implant in the three groups.At 2 and 4 weeks postoperatively,compared with the other two groups,the new bone of the NGF/GS group formed earlier,the bone density was higher and the percentage of new bone was greater than the other two groups.The percentages were as follows: the NGF/GS group at 2 and 4 weeks were(33.728±6.377)% and(55.854±6.842)%,the GS control at 2 and 4 weeks were(22.139±4.346)% and(46.047±3.998)%,the control group at 2 and 4 weeks were(19.809±3.105)% and(39.250±6.151)%.At 8 weeks postoperatively,the bone regenerated completely in all the three groups,and the bone density was almost the same.Conclusions: NGF/GS composites could accelerate bone regeneration in cavity bone defects of rabbit tibias,This may suggest that suggests the application of NGF may shorten shotern the healing time for bone defect.
