自体骨复合旋转铰链膝关节治疗骨巨细胞瘤初步研究

Objective To investigate the clinical feasibility of rotating - hinge knee autograftprosthetic composite for treatment of giant cell tumors.Methods Seven patients with giant cell tumor adjacent to the knee joint, comprising 5 males and 2 females with a mean age of (30.5±9.57) years old,received knee joint replacement with rotating-hinge knee autograft-prosthesis composite since January 2006.Of these lesions, five located at the distal femur and 2 at the proximal tibia.Three patients were with primary tumor and 4 with recurrent tumor.Pathological fracture was complicated in 3 patients with Campanacci stage Ⅲ disease.During operation, extensive tumor resection was performed, and rotating-hinge knee autograftprosthetic composite was used for repair of tumor-related bone deficiency.Results During the follow-up for 12 to 36 months, no local relapse, metastasis, loosening or rupture of prosthesis were found in these 7 cases.The mean time for healing between autograft and host bone was (5±1.3) months.Active motion range of postoperative knee were 70°to 110°, with a mean of (901±4.82) ° Conclusion Rotating-hinge knee autograft-prosthetic composite is feasible for giant cell tumor that necessitates replacement of knee joint, and its long-term efficacy needs to be followed up.
