代谢综合征患者颈动脉硬化与同型半胱氨酸相关性研究

摘    要:目的:探讨在代谢综合征(MS)患者中,颈总动脉平均和最大内膜中层厚度(IMT),颈总动脉扩张性、紧张度和僵硬度与同型半胱氨酸的相关性。方法:研究对象包括33名正常人(对照组)、76例非MS患者(非对照组)和83例MS患者(MS组)。所有研究对象均采用彩色多功能超声诊断仪检测颈总动脉的收缩和舒张内径,并计算动脉的扩张性、紧张度和僵硬度,检测颈总动脉平均和最大IMT,并检测血清同型半胱氨酸水平。结果:代谢综合征患者血清同型半胱氨酸及颈总动脉平均和最大内膜中层厚度均明显高于正常对照组和非MS组。与对照组和非MS组比较,MS患者颈总动脉扩张性、紧张度明显下降(P〈0.01);僵硬度明显升高(P〈0.01);平均及最大IMT增加(P〈0.05);平均和最大IMT分别与血清同型半胱氨酸独立相关(P〈0.01),颈总动脉的扩张性和紧张度与血清同型半胱氨酸独立负相关(P〈0.01),颈总动脉的僵硬度与血清同型半胱氨酸独立相关(P〈0.01)。结论:代谢综合征患者颈动脉硬化加剧,并与血清同型半胱氨酸独立相关,提示血清同型半胱氨酸在动脉粥样硬化中有一定的作用。

Abstract:Objective:To assess the association between atherosclerotic progression,estimated by common carotid intimamedia thickness (IMT),distensibility,strain and stiffness,and serum levels of hyperhomocysteinemia (HHcy) in subjects with metabolic syndrome (MS).Methods:33 normal subjects,76 samples with non-MS and 83 patients with MS were enrolled.Serum homocysteinemia was measured;and mean and max common carotid IMT were determined,and the systolic and diastolic diameters of common carotid and carotid intima-media thickness (IMT) were measured in supine position in the left and right common carotid.The arterial distensibility,arterial strain and arterial stiffness were calculated. Results:The arteriosclerosis mesured by arterial distensibility,arterial strain,arterial stiffness and mean and max IMT of common carotid artery were significantly aggravated in MS group compared with control group and non-MS group (P0.01),Serum homocysteinemia levels in MS group were significantly elevated compared with control and non-MS group (P0.05 or 0.01).Arterial stiffness and mean and max IMT of common carotid artery positively correlated with serum homocysteinemia levels independently ( P0.01) while arterial distensibility and arterial strain negatively correlated with serum homocysteinemia levels independently ( P 0.01).Conclusion:In the patients with MS,the present results indicate that hyperhomocysteinemia possibly participates in the therogenic process.
