亚低温治疗对心肺复苏后患者神经功能影响的临床研究

摘    要:目的探讨心肺复苏后亚低温治疗对神经功能的影响。方法选择2008年4月至2013年7月在梅州市人民医院接受诊治的心肺复苏患者46例作为研究对象,按照是否采用亚低温治疗将患者分为亚低温组(n=23)与常温组(n=23)。比较两组患者的良好转归率、监测指标以及神经功能评分。结果亚低温组的良好转归率显著高于常温组(P<0.05);治疗7 d后亚低温组的道格拉斯昏迷评分、平均动脉压、血液氧饱和度、pH与常温组比较差异无统计学意义(P>0.05),但体温显著低于常温组(P<0.05);亚低温组神经功能缺损评分显著高于常温组,而改良Barthel指数评分显著低于常温组(P<0.05)。结论亚低温治疗对心肺复苏后脑复苏患者神经功能具有明显的保护作用,改善患者神经功能状况。

Abstract:Objective To investigate the effects of hypothermia on neurological function after cardiopulmonary resuscitation(CPR). Methods Total of 46 CPR patients in Meizhou City People's Hospital from Apr. 2008 to Jul. 2013 were included, the patients were divided into mild hypothermia group( n = 23 ) and normal temperature group( n = 23 ) according to whether low temperature therapy was applied. Good outcome rate, monitoring indicators and neurological function of the two groups were compared. Results Good outcome rate of mild hypothermia group were significantly higher than normal temperature group ( P 〈 0.05 ). After 7 d,GCS,MAP,SaO2 and pH of the two groups had no statistically significant difference( P 〉0.05 ), but the temperature of mild hypothermia group was significantly lower than the normal temperature group ( P 〈 0.05 ). NFI scores of mild hypothermia group were significantly higher than normal temperature group, and MBI scores were significantly lower than normal temperature group (P 〈 0.05 ). Conclusion Hypothermia can protect the neurological function of cerebral resuscitation patients after CPR, and improve the neurological conditions.
