踝关节骨折的手术治疗和手法复位治疗效果比较分析

目的比较手法复位和手术治疗两种方法治疗踝关节骨折的临床疗效,为踝关节骨折治疗方法的选择提供依据。方法选取2007年12月至2013年6月北京密云县医院收治的踝关节骨折患者106例,根据患者自愿和病情情况,将106例踝关节骨折患者分为手术组(50例)和手法复位组(56例)。比较两组患者的复位情况。结果手法复位组骨折复位优良率和踝关节功能恢复优良率分别为82.14%和87.50%,均显著低于手术治疗组的96.00%和94.00%(Z=8.947,2.989,P均<0.05)。手法复位组平均住院天数为(5.5±3.4)d,平均住院费用为(2503.1±534.6)元,并发症发生率为3.57%,显著低于手术治疗组(P<0.05)。结论手术治疗和手法复位治疗踝关节骨折各有优缺点,在具体治疗中,需要根据患者情况和意愿选择合适的治疗方法。

The Comparative Analysis of Manipulation Reduction and Operation Therapy in the Treatment of Ankle Fracture

Objective To compare the clinical effects of manipulative reduction and operation therapy treating ankle fracture, and provide a basis for selecting reasonable method to treat ankle fracture. Methods During Dec. 2007 and Jun. 2013 ,a total of 106 patients with ankle fracture in Beijing Miyun Hospital were selected, according to patients＇ condition and patients＇ will, they were divided into operation group（50 cases） and manipulation group （56 cases）. The reduction conditions of the two groups were compared. Results The excellent and good rate of fracture reduction and ankle function recovery were 82.14% and 87.50% in manipulation group which were significantly lower than that （96.00% and 94.00% ） in the operation group （ Z = 8. 947,2. 989, both P 〈 0.05 ）. The average hospitalization time was （5.5 ± 3.4） d, the average hospitalization expenses was （2503.1 ± 534.6 ） yuan and the incidence of complications was 3.57 % in manipulation group, significantly lower than that in the operation group （ P 〈 0.05 ）. Conclusion The operation therapy and manipulative reduction for ankle joint fracture both have their own advantages and disadvantages. The choice of treatment should be based on patients＇ condition and willingness.
