摘 要: | 目的探讨幽门螺杆菌(Hp)感染与婴幼儿缺铁性贫血(IDA)的关系及铁剂联合Hp根除治疗对婴幼儿IDA的影响。方法选择2010年110月临潼区妇幼保健医院收治的IDA婴幼儿238例作为研究组,并按入院先后顺序分为单纯铁剂治组(110例)与铁剂联合Hp根治组(128例),另外选取100例健康婴幼儿作为正常对照组。比较研究组及正常对照组婴幼儿Hp的感染率;检测单纯铁剂治组和铁剂联合Hp根治组患儿治疗前后血清铁、血清铁蛋白、Hp-IgG及炎性因子的变化。结果研究组血红蛋白、转铁蛋白饱和度、血清铁蛋白均低于正常对照组,血清铁、转铁蛋白水平和Hp感染率高于正常对照组,差异均有统计学意义(P<0.01)。治疗8周后随访检测Hp-IgG滴度变化,铁剂联合Hp根治组血清Hp-IgG滴度较单纯铁剂治组显著降低(P<0.05)。治疗8周后,铁剂联合Hp根治组与单纯铁剂治组患者血红蛋白水平显著上升,血清铁水平显著下降,且铁剂联合Hp根治组优于单纯铁剂治组(P<0.01)。结论 IDA婴幼儿Hp感染率显著高于非IDA儿童,铁剂联合Hp根除治疗对Hp感染婴幼儿IDA治疗有益。

Abstract: | Objective To investigate the relationship between Helicobacter pylori (lip) infection and iron deficiency anemia(IDA) in infants and young children and the effect of Hp eradiatian on infants and young children IDA. Methods A total of 238 infants and young children patients with IDA diagnosed by Lintong District Maternal and Child Health Hospital from Jar. 2010 to Oct. 2013 and 238 cases of healthy infants were included in the study, who were randomly divided into the chalybeate treatment group and the chalybeate combined anti-Hp eradication treatment group according to the admission sequence;and another healthy children were selected as the control group. The Hp infection rate of the study groups and control group was compared, serum levels of iron, ferritin,Hp-IgG and inflammatory cytokines of the chalybeate treat- ment group and the chalybeate combined anti-Hp eradication treatment group were detected before and after treatment. Results The hemoglobin, serum iron, transferrin saturation, serum ferritin were lower than those in normal control group, levels of transferrin and tip infection rate were higher than the normal control group, the differences were statistically significant( P 〈 0.01 ). After 8 weeks of treatment, the follow-up testing of the titer of Hp-IgG changes indicated that serum Hp-IgG of chalybeate combined with lip eradication treat- ment group was significantly lower than pure chalybeate treatment group ( P 〈 0.05 ). After 8 weeks of treat- ment, hemoglobin level was significantly increased, serum iron level was significantly. decreased in both chalybeate combined with lip eradication treatment group and chalybeate treatment group, and the former group better than the latter group ( P 〈 0.01 ). Conclusion The infection rate of Hp in the infants and young children with IDA is significantly higher than that of the non-IDA infants and young children. Chalybeate combined Hp eradication therapy is beneficial for the IDA of tip infected infants and young children. |
