胆总管结石乳头括约肌小切开联合气囊扩张术临床应用效果观察

目的研究胆总管结石乳头括约肌小切开联合气囊扩张术临床应用效果。方法选择2010年2月至2013年3月深圳市人民医院接诊的110例胆总管结石患者作为研究对象,按照随机数字表法分为试验组和对照组,各55例。对照组采用传统的乳头括约肌小切开术,试验组采用乳头括约肌小切开联合气囊扩张术对患者进行治疗,比较两组患者的取石成功率及近、远期并发症发生情况。结果试验组患者的结石清除率和机械碎石率与对照组比较差异无统计学意义(P>0.05)。试验组手术时间显著短于对照组(P<0.01),患者术后的近、远期并发症发生率均显著低于对照组(P<0.05)。结论应用胆总管结石乳头括约肌小切开联合气囊扩张术治疗效果良好,值得临床推广应用。

The Clinical Effect of Papillary Sphincter Small Incision Combination with Balloon Dilatation for Choledocholithiasis

Objective To study the clinical effect of papillary sphincter small incision combination with balloon dilatation for choledocholithiasis.Methods A total of 110 cases of choledocholithiasis from Shenzhen People＇s Hospital from Feb.2010 to Mar.2013 were included in the study.According to a random number table method,they were randomly divided into trial and control groups,55 cases in each group.The control group received conventional papillary sphincter small incision surgery,and the trial group received papillary sphincter small incision combined balloon dilatation treatment.After surgery,the stone taking success rate and short and long term complications of the two groups were investigated and analyzed.Results The stone clearance rate and mechanical lithotripsy of the trial group and control group had no statistically significant difference（P 〉 0.05）.The operation time of the trial group was significantly shorter than the control group（P 〈 0.01）.The postoperative and long term complication rate were significantly lower than the control group（P 〈 0.05）.Conclusion The clinical effect of papillary sphincter small incision combined with balloon dilatation for choledocholithiasis is good,and worthy of clinical application.
