多项指标联合诊断及鉴别诊断胰腺内分泌肿瘤与腺泡细胞癌的病理研究

目的探讨胰腺内分泌肿瘤(PET)的临床病理学特征及与腺泡细胞癌的鉴别诊断,以期提高临床治疗水平。方法随机选取2010年1月至2013年2月无锡市人民医院收治的10例PET患者为研究对象,从病理学、免疫组织学角度进行分析诊断,并比较其与腺泡细胞癌在镜检和免疫功能指标的差异性。结果胰腺内分泌肿瘤细胞学上分叶状,有纤维组织间隔,细胞丰富,免疫组织化学上表达胰蛋白酶、抗淀粉酶和少量神经元特异性烯醇化酶,超微结构上为不明显腺腔样结构,腔面可见微绒毛。结论 PET多数为良性肿瘤,而腺泡细胞癌多数为高度侵袭性肿瘤,两者在形态上类似,但是在生物学上明显不同。

Multiple Indexes Combined Diagnosis and Pathological Differential Diagnosis of Pancreatic Endocrine Tumors and Acinar Cell Carcinoma

Objective To investigate the pathological features of pancreatic endocrine tunor（PET） and differential diagnosis from acinar cell carcinoma,in order to improve the level of clinical treatment.Methods Randomly selected during Jan.2010 and Feb.2013 from Wuxi City People＇s Hospital,10 cases of PET were included in the study,analysis and diagnosis from pathological and immunohistochemical perspective was done and the differences in microscopy and immune function indexes from acinar cell carcinoma were compared.Results Pancreatic endocrine tumors cytology features were lobulated fibrous tissue gap,rich cells,immunohistochemistry features were the expression of trypsin,anti-amylase and a small amount of neuron-specific enolase,the ultrastructure features were the atypical glandular-like structure,microvilli visible on the cavity surface.Conclusion Most PET is benign,while the majority of acinar cell carcinoma is highly invasive tumor,the two of which are similar in morphology,but are significantly different in biology.
