原发性中枢神经系统淋巴瘤的诊断、治疗及预后分析

原发性中枢神经系统淋巴瘤(PCNSL)是一种十分罕见的淋巴结外的淋巴瘤,现阶段对于其发病机制尚有争论。其病理形态多为弥漫型大B细胞来源,影像学多表现为单发或多发的位于深部脑实质或血管周围的病变。CT与磁共振成像对于淋巴瘤的确诊意义较大,需要慎重对待及鉴别。脑脊液及骨髓检查对该病的确诊意义不大。PCNSL的临床表现与颅内其他肿瘤无显著性差异。治疗方案多为化疗联合放疗,可提高有效率,该病的预后受多种因素影响。

The Diagnosis,Treatment and Prognosis of Primary Central Nervous System Lymphoma

Primary central nervous system lymphoma （PCNSL） is a very rare extranodal lymphoma, at this stage there is still controversial for its pathogenesis. The lymphoma pathological morphology is mostly diffuse large B cell sourced, and the imaging manifestation is mostly solitary or multiple lesions located in deep brain parenehyma or peripheral vaseulature. CT and MRI have a great significance in the diagnosis of lymphoma, but careful treatment and differential diagnosis are needed. Cerebrospinal fluid and bone marrow examination have little value in the diagnosis of lymphoma. The clinical manifestationsof PCNSL have no significant differences from other intraeranial tumors. Treatment programs, are mostly chemotherapy combined radiotherapy, which can improve the efficacy. The prognosis depends on a variety of factors.
