玻璃体切除联合腔内注药治疗晶状体植入术后眼内炎的临床研究

目的探讨玻璃体切除联合腔内注药治疗晶状体植入术后眼内炎的临床效果。方法选择2007年5月至2013年5月成都市第三人民医院眼科收治的行白内障人工晶体植入术后发生眼内炎的患者60例,根据入院先后排序分为两组:对照组29例实施常规腔内注药治疗,观察组31例在对照组的基础上给予玻璃体切除术。对两组患者治疗前后的视力进行统计,评价两组患者的治疗效果,炎症消失时间及并发症发生情况。结果经过治疗,观察组患者视力显著优于对照组,治疗总有效率显著优于对照组,炎症消退时间显著短于对照组,且并发症发生率显著低于对照组,差异均有统计学意义(P<0.05)。结论晶状体植入术后眼内炎是眼科急重症,玻璃体腔内注射联合早期玻璃体切除术对于改善患者预后有重要意义,一旦确诊应立即给予玻璃体腔内注射,并应尽早行玻璃体切除术,以更快地控制炎症,保护患者眼结构和功能。

Clinical Studies on Vitrectomy Combined with Cavity Injection Treatment of Postoperative Endophthalmitis after Lens Implantation

Objective To investigate the clinical effect of vitrectomy combined with cavity injection therapy endophthalmitis after lens implantation. Methods From Chengdu Third People＇s Hospital during May 2007 and May 2013 60 patients with endophthalmitis after cataract IOL implantation were included in the study, and were divided into two groups randomly according to admittance sequence of odd and even numbers ： control group （ 29 cases） treated with conventional endovascular drug injection ; observation group （31 cases） were treated with vitrectomy on the basis of the control group＇s treatment. Visual acuity before and after treatment of the two groups were recorded, the therapeutic effect, inl＇lan~mation disappearance time and complications of the two groups were assessed. Results After treatment,visual acuity of the patients in observation group was significantly better than the control group, the total effective rate was better than the control group, the inflammation disappearance time was significantly shorter than the control group, and the incidence of complications was significantly less than the control group, the differences were all statistically significant（ P 〈 O. 05 ）. Conclusion Endophthalmitis after lens implantation is a severe acute ophthalmolo- gy,intravitreal injection combined with early vitrectomy is of great significance for improving the prognosis of patients, once diagnosed intravitreal injection should be immediately given, and vitrectomy should be performed as soon as possible, so as to control inflammation quickly and protect the eye structure and func- tions of the patient.
