鼻咽癌组织中的癌胚抗原

摘    要:应用未标记免疫酶技术(PAP法)显示94例鼻咽癌和52例慢性鼻咽炎组织切片中的癌胚抗原(CEA)。鼻咽癌的CEA阳性率为28.7%,分化差的腺癌CEA阳性率(53.8%)显著高于分化差的鳞癌(17.6%),未分化癌失去产生CEA的能力。CEA含量与癌瘤分化程度有关。分化好的鳞癌角化区域和慢性鼻咽炎中正常鳞状上皮CEA阳性反应可能是抗CEA血清与角质或前角质物质交叉反应的结果。鼻咽假复层纤毛柱状上皮CEA阳性率为35.7%(15/42),似与鼻咽的粘膜炎症轻重有关,本组轻度炎症者仅10例CEA均为阴性。本文还对鼻咽癌组织中CEA出现的生物学意义作了初步分析。

Abstract:94 cases of nasopharyngeal carcinoma (NPC) and 52 nasopharyngitis have been studied immunohistochemically for the presence of carcinoembryonic antigen (CEA) within tumor cells and nasopharyngeal epiethelial cells. 28. 7% of the NpC was positive for CEA. The relationship between the presence of CEA and the histological type and diffirentiation of the tumors was investi- gated. The positive incidence of CEA. in poorly-differentiated adenocarcinoma of nasopharynx was much higher (53. 8%) than that of poorly-differentiated squamous type (17. 6%) and no CEA was found in undifferentiated ones, The positivs reaction seen in well-differentiated squmous-cell carcinoma, especia- lly in the cornified components and that in the squamous epithelium of chr- onic nasopharyngitis was explained as a cross-reacting phenomenon from Ker- atin or Keratis precursor. Lymphocytic infiltration in the tumor did not show any influence on the presence of CEA. pseadostratified columnar epithelium was also positive for CEA in 15/42 (35. 7%). Its close association with inflammation in the nasopharynx was obvious as no CEA was demonstrated in a series of mild nasopharyngitis 0/10 (0%) but increased while inflammations became prominent 15/32 (46. 9%). Thus, CEA, as a tumor marker, was considered to be non-specific but its biological significance in prognosis of NPC has been emphasized.
