室间隔完整型肺动脉闭锁的超声诊断及临床评估

目的探讨室间隔完整型肺动脉闭锁(PA IVS )的超声表现及探测要点,并依据结果作初步临床评估.方法收集13例经心血管造影证实的PA IVS患者的超声心动图资料,并与手术和临床随访结果进行对比,随访时间6～15个月.结果 13例患儿中11例在首次超声心动图检查中得到确诊,另2例误诊.有 8例进行随访,结果显示:3例未经手术早期死亡,2例手术治疗,其中1例存活,1例死亡;3 例未手术存活至今.结论超声心动图对PA IVS具有较高的诊断价值.在超声检查中不仅仅是做出诊断,观察右心室发育情况(三尖瓣环直径)对该类疾病的临床评估也具有重要意义, 有助于指导外科手术并估计预后.

Echocardiographic Diagnosis and Clinical Assessment of Pulmonary Atresia with Intact Ventricular Septum

Objective To evaluate the echocardiographic features and diagnostic accuracy in patients with pulmonary atresia with intact ventricular septum(PA IVS),and to assess it's clinical prognosis.Methods The echocardiographic data of 13 patients with PA IVS confirmed by cardiovascular angiography were collected,and then compared with the results of surgery and follow-up the time of which varied from 6 to 15 months.Results Of the 13 patients,11 patients were diagnosed accurately by the first time of echocardiography,other 2 patients were misdiagnosed. Eight patients were followed up from 6 to 15 months:3 patients died without operation;2 patients underwent operations,one died,and the other survived;Other 3 patients without operation have still survived.Conclusions Echocadiography plays a very important role in diagnosing patients with PA IVS,in assessing their clinical prognosis, and for choosing operation methods according to the right ventricular development by the measurement of the tricuspid diameters.
