胃-十二指肠溃疡穿孔修补术临床疗效分析

目的探讨采用单纯缝合修补术治疗胃、十二指肠溃疡急性穿孔的临床疗效,并总结手术经验。方法对2008-09～2011-03间,收治的42例胃、十二指肠溃疡急性穿孔的患者,行开腹探清病灶后,着重行单纯穿孔修补术,并酌情行Ⅰ期胃大部分切除术。结果本组42例患者中,行单纯缝合修补术39例,Ⅰ期胃大部分切除术3例。住院时间10～21 d,平均14.5 d。本组患者无死亡、无并发症发生。38例患者获得随访6～24个月,随访率90.48%。38例患者均做胃镜或钡餐检查,其中:34例治愈,治愈率89.47%;4例患者复发,复发率10.52%。4例中,3例患者出现溃疡病一般症状,继续口服抗溃疡药物后症状减轻或改善,无需再手术治疗;1例经胃镜等检查为胃窦部溃疡复发,因瘢痕大,行胃大部切除术后痊愈。结论胃、十二指肠溃疡穿孔修补术,具有创伤小、恢复快、并发症少,且能阻断病情继续发展的优点,术后继续进行正规药物治疗可获得满意的临床效果。
