摘    要:目的探讨RET原癌基因在甲状腺结节细针穿刺标本中的表达及其对良恶性诊断的意义。方法选择我院普外科拟行手术切除甲状腺结节的住院患者65例,术前行甲状腺结节细针穿刺(FNA),细针穿刺涂片做RET免疫组化染色。以手术切除的甲状腺结节组织标本的病理诊断做为黄金标准,评价RET原癌基因在甲状腺结节良恶性诊断中的意义。结果依据术后病理诊断,良性甲状腺结节30例,恶性甲状腺结节35例(甲状腺乳头状癌28例,甲状腺滤泡状腺癌7例)。RET在甲状腺恶性结节FNA样本中的阳性率高于甲状腺良性结节中的阳性率(82.35%VS 10%,χ2=34.31,P<0.05),与普通甲状腺FNA相比,结合RET特殊染色可提高结节良恶性诊断的敏感度和特异度(82.86%VS 65.71%;90%VS 86.67%)。结论 RET在甲状腺恶性结节的阳性率高于甲状腺良性结节,FNA结合RET特殊染色可提高术前甲状腺结节良恶性诊断的敏感度和特异度。

RET Proto-oncogene Expression in Fine Needle Aspiration of Thyroid Nodules

Abstract:Objective To investigate the RET proto-oncogene expression in thyroid nodule fine needle aspiration and the significance of the diagnosis of benign and malignant. Methods 65 cases who were underwent surgical removal of thyroid nodules in our hospital were studied.Preoperative thyroid nodule fine-needle aspiration(FNA) was performed.Fine needle aspiration smears were detected by RET immunohistochemical staining.The pathological diagnosis was as the gold standard.To Evaluate RET proto-oncogene in the diagnosis of malignant and benign thyroid nodule. Results According to the postoperative pathological diagnosis,30 cases of benign thyroid nodules,35 cases of malignant thyroid nodules(28 cases of papillary thyroid carcinoma,thyroid follicular carcinoma n=7).Positive rate of RET in the malignant thyroid nodule FNA samples is higher than in benign thyroid nodules(82.35% vs 10%,χ2 34.31,P<0.05).Compared with normal thyroid FNA,combined with the special staining of RET can improve the sensitivity and specificity of the nodules of benign and malignant diagnosis(82.86% vs.65.71%;90% vs.86.67%). Conclusion The positive rate of RET in thyroid malignant nodules was higher than in benign thyroid nodules.The FNA combined with special staining of RET could improve the sensitivity and specificity of preoperative malignant diagnosis of benign thyroid nodule.
