锁定加压钢板治疗高龄桡骨远端粉碎性骨折

摘    要:目的探讨切开复位锁定加压接骨板(LCP)内固定治疗桡骨远端粉碎性骨折的效果及其评价。方法回顾分析2005-03~2008-06间,对46例桡骨远端粉碎性骨折采用掌侧入路切开复位(LCP)内固定进行治疗。其中:14例给予植骨,12例术后用掌侧石膏托支持。结果术后所有患者随访6~30个月(平均21个月),所有骨折均愈合。桡骨远端的掌倾角平均9.7°;尺偏角平均21.3°;桡骨短缩≥2.0 mm 4例,其余为<2.0 mm;关节面移位均<1.0 mm。用改良的Gartland和Werley评分系统评估腕关节功能,优32例,良11例,中2例,差1例,优良率为93.5%。结论切开复位掌侧(LCP)内固定是治疗桡骨远端粉碎性性骨折的有效方法,辅以植骨能有效防止关节面塌陷和促进骨折愈合。

Abstract:Objective To open reduction and locking compression plate(LCP) within a fixed treatment of distal radius comminuted fracture and evaluate its effect.Methods From March 2005 to June 2008,46 cases of distal radius comminuted fractures were treated with volar LCP fixation.14 cases of bone grafting,12 cases of postoperative volar plaster care support.Results All patients were followed up for 6~30 months(mean 21 months),all fractures healed.The inclination of the distal radius of palm average of 9.7°;ulnar deviation of the average 21.3°;4 cases radial shortening ≥2.0 mm,the rest are <2.0 mm;displacement of the articular surface <1.0 mm.Modified Gartland and Werley scoring system were used to assess the function of the wrist,excellent in 32 cases,good in 11 cases,2 cases and poor in 1;excellent rate was 93.5%.Conclusion Open to reset the palm side LCP fixation is a comminuted fracture of the distal radius,supplemented with bone graft can effectively prevent the articular surface collapse and promote fracture healing.
