急性前交叉韧带损伤的MRI检查临床分析

目的探讨MRI检查在急性前交叉韧带损伤患者诊断中的临床实用价值。方法对我们医院的75例急性创伤性膝关节血肿的患者进行前抽屉试验及Lachman试验的两项临床检查和MRI的检查,最后进行关节镜的检查。以关节镜的检查结果为判断依据,对临床检查和MRI检查诊断急性前交叉韧带损伤的准确率进行比较分析。结果经关节镜分析得出75例急性创伤性膝关节血肿患者中急性前交叉韧带损伤53例,正常22例;临床检查共发现急性前交叉韧带损伤真阳性数28例,真阴性数21例,假阳性数5例,假阴性数21例,准确度为65.3%;MRI诊断结果为真阳性数48例,真阴性数23例,假阳性数1例,假阴性数3例,准确率为94.7%;MRI对于急性前交叉韧带损伤的诊断准确率明显高于临床检查,具有统计学意义(P<0.01)。结论 MRI检查能较准确对急性前交叉韧带损伤作出诊断,其诊断准确率明显优于临床检查,应成为临床检查急性前交叉韧带损伤早期诊断的首选检查手段。

The Clinical Analysis of MRI Examination of Acute Anterior Cruciate Ligament Injury

Objective To explore the clinical value of MRI examination in patients with acute anterior cruciate ligament injury. Methods Take two clinical examination of drawer test and Lachman conducted tests and MRI examination for 75 cases of acute traumatic knee hematoma anterior the patient in our hospital, and then the ifnal check arthroscopy. Check the results of arthroscopic judgment based on pre-clinical examination and MRI diagnostic accuracy of acute ligament injury crossover comparative analysis. Results The analysis of results 75 cases of arthroscopic knee hematoma in patients with acute traumatic acute anterior cruciate ligament injury in 53 cases, 22 cases of normal;clinical examination were found in acute anterior cruciate ligament injury really positive in 28 cases, 21 cases of true negatives , 5 cases of false-positive and false-negative number of 21 cases , the accuracy was 65.3%;MRI diagnosis was true positive in 48 cases, the number of true negative in 23 cases, one case of false-positive and false-negative number three cases, the accuracy was 94.7%. MRI for diagnostic accuracy of acute anterior cruciate ligament injuries was signiifcantly higher than clinical examination,with statistical signiifcance （P＆lt;0.01）. Conclusion MRI examination can accurately make a diagnosis of acute anterior cruciate ligament injury.The diagnostic accuracy was superior to clinical examination. Clinical examination should be the main choice before the early diagnosis of acute ligament damage in cross examination.
