中医情志护理对老年肝癌病人不良情绪和生存期的前瞻性对照研究

目的]探讨中医情志护理对老年肝癌病人不良情绪和生存期的影响。方法]前瞻性选取老年原发性肝癌病人86例,随机分为研究组和对照组各43例。对照组行常规临床护理,研究组在对照组的基础上进行中医情志护理。比较两组护理前、护理1周和护理2周的焦虑自评量表(SAS)、抑郁自评量表(SDS)得分、住院天数、病人满意度和并发症发生率,并采用Kaplan-Meier生存曲线分析比较两组随访半年和1年的生存率。结果]与对照组比较,研究组护理1周和护理2周的SAS和SDS得分均降低,住院天数缩短,病人满意度和随访1年的生存率提高(P0.05);与护理前比较,研究组护理1周和护理2周的SAS和SDS得分均降低,对照组护理2周的SAS和SDS得分亦均降低(P0.05);两组并发症发生率比较差异无统计学意义(P0.05)。结论]在常规护理的基础上进行中医情志护理有助于改善老年原发性肝癌病人的不良情绪和生存期。

Prospective controlled study on TCM emotional nursing on the bad mood and survival period of elderly patients with liver cancer

Objective:To probe into the prospective controlled study of TCM emotional nursing on the bad mood and survival period of elderly patients with liver cancer.Methods:A total of 86 cases of elderly patients with primary liver cancer in our hospital from June 2013 to June 2015 were selected and randomly divided into the research group (n=43) and the regular group (n=43).Regular group had routine clinical care, and the research group had traditional Chinese medicine accommodating emotion nursing.Before nursing,after nursed for 1 week and 2 weeks self-evaluation of anxiety scale(SAS)score,depression self rating scale(SDS)score,hospitalization days,patient satisfaction rate and complications of two groups were compared,and Kaplan Meier survival curve was applied in analysis and comparison of the survival rate of two groups after followed up for six months and 1 year.Results: Compared with control group,after nursed for 1 week and 2 weeks SAS and SDS scores of the research group were lower,hospitalization days of the research group was shorter,patient satisfaction rate and followed up 1 year survival rate of the research group were increased (P<0.05).Compared with before treatment,after nursed for 1 week and 2 weeks SAS and SDS scores of the research group were lower,and after nursed for 2 weeks SAS and SDS scores of the control group were also lower (P<0.05).Complication rates of two groups had no statistical significant differences (P>0.05).Conclusion: Traditional Chinese Medicine emotional nursing on the basis of the conventional nursing could help improve bad mood and survival conditions of elderly patients with primary liver cancer,therefore,it was worthy of clinical popularization and application.
