行为疗法对肥胖型2型糖尿病自我管理行为及代谢指标的影响

[目的]探讨行为疗法对肥胖型2型糖尿病病人的自我管理行为及代谢指标的影响。[方法]将106例肥胖型2型糖尿病病人分为对照组和干预组各53例。对照组接受常规护理、糖尿病健康教育及门诊随访;观察组在此基础上予以行为疗法,包括病人评估及健康教育、饮食治疗、运动治疗、日记疗法、基于微信平台随访管理。比较两组病人在干预前及干预后3个月、6个月时自我管理行为的改变,同时比较两组代谢指标的差异,包括空腹血糖、餐后2h血糖、糖化血红蛋白、体重、体重指数、腰围、腰臀比、甘油三酯、低密度脂蛋白、高密度脂蛋白。[结果]干预后3个月、6个月干预组自我管理行为问卷评分和代谢指标显著高于对照组(P0.001,P0.05)。[结论]行为干预能有效提高病人自我管理能力,改善病人的代谢指标。

Influence of behavioral therapy on self management behavior and metabolic markers of type 2 diabetes mellitus with obese

Objective:To probe into the influence of behavioral therapy on self management behavior and metabolic markers of type 2 diabetes with obesity.Methods:A total of 106 cases of type 2 diabetes with obesity were divided into control group and observation group(n=53).The patients in observation group accepted routine nursing,diabetes health education and outpatient follow-up.While on the basis the patients in observation group adopted the behavioral therapy,including patient evaluation and health education,diet therapy,exercise therapy,diary,follow-up management based on the platform of We Chat.The Chinese version scale of the Diabetes Self-Care Activities(CDSCA) and FPG,2hPBG,body weight,BMI,waist circumference,WHR,TG,LDL-C,HDL-C were adopted to evaluate the changes of the self-management behavior and metabolic markers before intervention ,after the intervention for 3 months and 6 months.Results:The score of the Chinese version scale of the Diabetes Self-Care Activities and metabolic markers of patients in observation group were significantly higher than those in control group after the intervention for 3 months and 6months.Conclusion:Behavioral intervention could effectively improve patient self-management ability and improve the patients'metabolic markers.
