实时三维超声心动图评价左心病变相关的肺高压患者右心室形态和收缩功能

目的应用实时三维超声心动图探讨左心病变相关的肺高压(pulmonary hypertension,PH)患者右心室形态和收缩功能的特征。方法入选左心病变相关的PH患者23例和健康对照者27例,行常规超声心动图检查,采集右心室实时三维超声心动图图像并进行定量分析,获得右心室整体和流入道部、体部、流出道部三个局部的容积及收缩功能的参数指标,与正常对照组进行比较,并进一步分析三维超声参数与常规超声心动图参数的相关性。结果 PH组右心室整体、流入道部、体部、流出道部的舒张末容积(EDV)、收缩末容积(ESV)均显著大于对照组(均P<0.001);右心室整体、流入道部、体部射血分数(EF)均显著小于对照组(均P<0.01),而流出道部EF值与对照组之间差异无统计学意义(P>0.05);右心室流入道部和体部每博输出量(SV)与对照组差异无统计学意义(P>0.05),而整体与流出道部SV略高于正常对照组(P<0.05)。PH组右心室整体ESV和流入道部ESV与肺血管阻力之间呈显著正相关(r=0.69,P<0.001;r=0.62,P<0.001),右心室整体EF和流入道部EF与肺血管阻力之间呈显著负相关(r=-0.72,P...
