腔内激光治疗术治疗下肢浅静脉曲张

目的探讨静脉腔内激光治疗术治疗下肢浅静脉曲张的安全性和有效性。方法回顾分析240例(282条肢体)下肢浅静脉曲张患者应用DIOMED 810nm激光治疗仪的临床治疗效果及近期随访结果。结果该组所有患者术后均恢复良好，临床症状消失，无复发。经1～12个月随访观察，治疗静脉均完全闭塞，无再通发生。结论静脉腔内激光治疗术治疗下肢浅静脉曲张安全、有效、费用少；作为一种新的微创技术有望取代传统的开放手术。

Endovenous laser treatment of varicose veins in lower extremities

To evaluate the safety and efficacy of endovenous laser treatment(EVLT), a novel technique for the minimally invasive treatment of the incompetent great saphenous veins (GSV) and superificial varicose veins to use laser energy, delivered endovenously via a sterile laser fiber placed inside the vein. 282 lower extremities in 240 patients with varicose veins were treated endovenously with pulses of 810nm laser energy through a bare-tipped laser fiber. Vein access for endoluminal placement of the fiber through a catheter was achieved by means of percutaneous puncture.Exact placement of the fiber was determined by direct visualization of the red aiming beam of the laser fiber through the skin. The clinical results of EVLT were analysed retrospectively. All the patients have excellent recovery after EVLT, and there was no recurrence in this group.At 1~12 months follow-up, all the 282 treated GSV segments were closed (100%) by initial treatment with no flow detectable by Doppler interrogation. [Conclusions] EVLT of the incompetent GSVs and superificial varicose veins appears to be a very effective and safety way. It may be a good alternative treatment to surgery, which is more expensive, involves a longer recovery and can leave scars.
