加长柄双动半髋关节置换治疗高龄股骨粗隆间不稳定骨折

目的:探讨加长柄双动半髋关节置换治疗高龄股骨粗隆间不稳定骨折的临床疗效。方法:2007年8月—2011年12月采用加长柄双动半髋关节置换治疗75岁以上股骨粗隆间不稳定骨折55例,平均随访20.3个月,观察术后并发症、下地时间及髋关节功能。结果:55例均获随访,无一例死于术后并发症,无深静脉血栓及脑型脂肪栓塞并发症,无髋关节脱位,平均下地时间7.5d,术后12个月Harris髋关节评分:优16例,良32例,可3例,差4例。结论:加长柄双动半髋关节置换术是治疗高龄股骨粗隆间不稳定骨折的一种安全有效的方法,术后恢复快,可早期负重,避免卧床并发症。

The long-stem bipolarprosthetic hip hemiarthroplasty for patients with unstable intertrochanteric fracture

Objective:To investigate the clinical effect of the long-stem bipolarprosthetic hip hemiarthroplasty for patients with unstable intertrochanteric fracture.Methods:From August 2007 to December 2011,55 patients more than 75 years old with unstable intertrochanterie fracture were treated with long-stembipolar hipprosthetic replacement.The period of follow-up was 20.3 months on average.The operational complication,ambulating timeand Harris hip-scoring were observed.Results:All patients were followed up.No patients died of operative complication.There was no deep vein thrombosis and cerebral fat embolism,without dislocation of the hip.The average time to ambulate after operation was 7.5 days.According to the Harris hip-scoring system.It was found excellent in 16 cases,good in 32 cages,poor in 3 cases,bad in 4 cases 12 months after operation.Conclusion:Hip hemiarthroplasty with long-stem bipolar is a safe and effective treatment for unstable intertrochanteric fractures in elderly patients.Patients have better prognosis.Early weight bearing and less complication.
