上皮细胞膜抗原在肾细胞癌和肾盂移行细胞癌中的表达

目的：了解上皮细胞膜抗原（EMA）在肾细胞癌及肾盂移行细胞癌不同临床分期和病理分级中的表达情况,寻求有助于鉴别二者的方法.方法：使用免疫组化方法检测67例肾细胞癌及24例肾盂移行细胞癌病人肿瘤中EMA表达.结果：EMA在肾细胞癌和肾盂移行细胞癌中均有明显阳性表达,肾颗粒细胞癌表达随临床分期升高而降低,肾盂移行细胞癌随临床分期的升高而升高,均有统计学意义（P＜0.05）;在T3、T4期肾透明细胞癌和肾盂移行细胞癌中,EMA的表达有显著性差异（P＜0.05）.结论：EMA在肾癌和肾盂癌中表达随细胞类型和分期分级不同而有差异,可在一定程度上反映肿瘤的恶性程度及预后;EMA可以作为鉴别高分期肾癌和肾盂移行细胞癌的一个依据.

Expression of epithelial membrane antigen in renal cell carcinoma and transitional cell carcinoma of renal pelvis

Objective: To detect the expression of epithelial membrane antigen (EMA) in renal cell carcinoma (RCC) and transitional cell carcinoma (TCC) of renal pelvis of different clinical stages and pathological classifications, and thus find out the approach to distinguishing these 2 carcinomas. Methods: EMA expression was detected in 67 patients with RCC and 24 patients with renal pelvic TCC by using immunohistochemical staining. Results: The positive EMA expression was found in both RCC and renal pelvic TCC. The positive rate of EMA in renal granular cell carcinoma decreased significantly with the progression of clinical stages ( P <0.05), but in renal pelvic TCC it increased significantly with the progression of clinical stages ( P <0.05). There was significant difference in EMA expression between renal clear cell carcinoma of T_3-T_4 stage and renal pelvic TCC ( P <0.05). Conclusion: EMA expression in RCC and renal pelvic TCC varies with cell type and tumor staging and classification, which, to some extent, indicates the malignant degree and prognosis of the tumor. EMA expression in RCC and renal pelvic TCC of advanced stages is significantly different, so it may serve as the evidence in the differential diagnosis.
