阴式全子宫切除术中阴道残端两角半荷包缝合与单纯锁边缝合的临床比较

目的: 探索阴式非脱垂子宫切除术中阴道残端缝合的最佳方法。方法: 收集该院2003年前后施行阴式非脱垂子宫切除病例160例。其中残端单纯锁边缝合100例, 残端两角半荷包缝合60例。均采用1 /0Dexon线。单纯锁边缝合法是从阴道两侧顶端开始前后阴道粘膜和前后腹膜4层一起缝合残端全程; 半荷包缝合法为两角半荷包缝合后再4层一起单纯锁边法缝合至中间并打结。两种方法阴道残端缝合的病例均于术后2个月回访, 由门诊医师检查阴道残端愈合和息肉发生情况。结果:单纯锁边缝合组100例, 回访79例, 息肉发生率为39 .24%; 半荷包缝合组60例, 回访45例, 息肉发生率为17. 78%, 两组比较有非常显著的临床意义(P<0. 01)。结论: 阴式非脱垂子宫切除术中残端缝合两角以半荷包缝合法优于单纯锁边缝合法。

Clinical comparasion of semi - pouch suture with simple locking stitch suture of vaginal, stump in vaginal total hysterectomy

Objective:To assess the clinical use of semi-pouch suture of vaginal stump in vaginal total hysterectomy.Methods:Clinical data of 160 non-prolapse vaginal total hysterectomy in 2003 were collected.100 cases were performed with simple locking stitch suture of vaginal stump.60 cases were performed with semi-ponch suture of vaginal stump. The rate of polypus of vaginal stump during 2 months postoperative were collected.Results:The rate of polypus of simple locking stitch suture group was 39.24%. That of the semi-pouch suture group was 17.7%.Conclusion:Semi-pouch suture is of greater clinical use compared with simple locking stitch suture of vaginal stump.
