MR与CT图像融合技术在帕金森病深部脑电刺激术后的应用

摘    要:目的 采用CT和MRI图像融合技术评价帕金森病(PD)患者深部脑电刺激(DBS)术后电极位置准确性的应用价值。方法 收集接受双侧DBS疗法的32例PD患者,采用术后颅脑薄层CT和术前MRI图像融合(融合组)技术并与常规术后复查MRI图像(常规组)比较,分析电极位置精确度及检查耗费时间。结果 融合组与常规组测量的电极位置相关性较好(P均<0.008);融合组与常规组电极尖端位置比较,除左侧电极y轴、z轴方向差异有统计学意义外(t=-2.34、-3.08,P均<0.05),余差异均无统计学意义(P均>0.05)。术后复查MR检查时间为(7.65±0.33)min,术后复查CT时间为(2.85±0.29)min,差异有统计学意义(P<0.05)。结论 DBS术后应用头部薄层CT与MRI图像融合能精确定位电极位置,并可避免PD术后复查MRI的潜在风险,缩短术后复查的时间。

Abstract:Objective To explore the accuracy of the electrodes position in deep brain stimulation (DBS) by using image fusion technique of post-operative CT with pre-operative MRI in patients with Parkinson Disease (PD).Methods Totally 32 patients with PD underwent bilateral DBS were acquired. The electrodes position and examination time of each patient were compared between fusion image of post-operative CT with pre-operative MRI (fusion group) and post-operative conventional MRI(control group).Results Fusion images of fusion group and conventional MR images of control group had high consistency in measure of the electrodes position (all P<0.008). The differences of the left electrodes position in the y and z axis had statistically significance between fusion group and control group (t=-2.34,-3.08; both P<0.05), others had no statistically significance (all P>0.05). Post-operative MRI examination time and post-operative CT examination time were (7.65±0.32)min and (2.85±0.292)min, respectively (P<0.05).Conclusion Application of post-operative CT and pre-operative MRI image fusion after DBS operation can accurately identify the electrodes position. It can reduce the potential risk encountered by post-operative MRI examination and shorten the post-operative scan time for patients.
