乳腺癌术后皮下积液的预防及处理

目的：探讨乳腺癌术后皮下积液发生的原因、预防方法及其处理措施。方法：我院2000年3月～2008年12月行乳腺癌改良根治术的282例患者中,35例发生皮下积液,对其临床资料进行回顾性分析。结果：282例患者发生皮下积液35例,占12.41%,其中,140例患者采用单纯腋窝引流、外加胸带加压包扎,发生皮下积液25例,占17.82%;142例患者采用腋窝及锁骨下、胸骨旁皮瓣下＂Y＂形负压双管引流、外加胸带加压包扎,发生皮下积液10例,占7.04%。两种不同的引流方法对乳腺癌术后皮下积液的发生率比较,有显著性差异（χ^2=7.585,P〈0.01）。结论：有效充分的负压引流、适当的加压包扎是预防皮下积液的重要措施。

The prevention and treatment of subcutaneous effusion after breast cancer operation

Objective：To investigate the influential factors,main causes of prevention and cure measures of subcutaneous effusion after breast cancer operation. Methods：282 cases of patients with breast cancer underwent radical operation of mastocarcinoma or reforming radical operation of mastocarc-inoma from March 2000 to December 2008 in our hospital and their clinical data was retrospectively analyzed. Results：Among 282 cases of patients,35 cases occurred accumulative liquid of subcutaneous,the incidence was 12.41%; 140 cases using the compression bandaging on chest wall with the simple closed continous suction drainage,25 cases occurred accumulative liquid of subcutaneous,the incidence was 17.82%; 142 cases using the compression bandaging on chest wall with ＂Y＂-shaped negative-pressure drainage,10 cases occurred accumulative liquid of subcutaneous,the incidence was 7.04%. Two kinds of different drainage methods on the incidence rate of postoperative subcutaneous effusion were compared,there was a significant difference （χ^2=7.585,P0.01）. Conclu-sion：Effective and adequate suction drainage,moderate pressurizes wraps are important to prevent subcutaneous effusion.
