磁共振内耳成像在儿童感音神经性耳聋中的应用

摘    要:目的:探讨磁共振内耳成像在儿童感音神经性耳聋(SNHL)中的应用价值.方法:搜集38例可疑内耳畸形所致SNHL患儿,使用Philips Gyroscan Intera 1.5 T超导磁共振仪,首先进行颅脑常规扫描,排除颅内其他病变后,分别进行3D/T2WI/TSE轴位和B-TFE的超薄斜矢壮位扫描,迷路病变采用MIP重建.结果:16例32耳显示听神经、迷路正常.2例3耳Michel畸型;6例12耳Mondini畸型;12例24耳前庭导水管扩大;20例畸形中有6例12耳同时伴有耳蜗前庭神经信号不同程度的缺失.2例4耳显示内听道狭窄伴蜗神经变细.结论:磁共振内耳成像对诊断儿童先天性SNHL有着重要的价值,对判断患儿内耳迷路及各神经发育情况有着不可替代的作用,是这类患儿进行人工耳蜗置换的术前必要检查.

Abstract:AIM: To investigate the value of MRI of inner ear in diagnosing infantile sensorineural hearing loss. METHODS: Thirty-eight infantile cases suspected as sensorineural hearing loss induced by inner ear malformation were examined by Philips Gyroscan Intera 1.5 T MR system . Routine sequences included conventional brain scan for excluding the cases of intracranial diseases, and 3D/T2WI/TSE axial scan and B-TFE oblique-sagital scan of inner ear. Labyrinth disease was reconstructed by MIP method. RESULTS: Sixteen cases(32 ears) showed normal structure of cochlear nerve and labyrinth; 2 cases (3 ears) presented Michel deformity ; 6 cases (12 ears) presented Mondini deformity ;12 cases(24 ears) showed vestibular aqueduct enlargement; 6(12 ears) of the 20 deformity cases were companied with absence of cochlear nerve to seme extent. Two cases(4 ears) showed cochlear nerve thinness and stenosis of inner ear canal. CONCLUSION: MRI of inner ear has a great clinical value in diagnosing infantile sensorineural hearing loss, because it can help to judge the station of labyrinth and nerves of inner ear.
