恶性淋巴瘤脾浸润化疗前后的超声显像分析

目的 分析超声显像对恶性淋巴瘤脾浸润患者在化疗前后的声像图特征。方法 对36例恶性淋巴瘤(其中何杰金病9例，非何杰金淋巴瘤27例)进行化疗前后的超声检查。结果 化疗前86．1％病例有脾脏肿大，所有病例脾内均有低回声病灶。声像图表现可分为三种类型：1．结节型；2．弥漫粟粒型；3．巨块型。本组以结节型多见，占69．4％。化疗后超声复查，大多数病例脾脏缩小，脾内浸润病灶消失。结论 超声显像对恶性淋巴瘤脾浸润化疗前的诊断和化疗后的监测具有重要的应用价值，可为临床诊断治疗和疗效评价提供无创的简便方法。

Ultrasonographic Analysis of Malignant Lymphoma with Spleen Infiltration Before and After Chemotherapy

Objective To analyze the ultrasonographic features of malignant lymphoma with spleen infiltration before and after chemotherapy.Methods Thirty six cases with malignant lymphoma(including nine Hodgkin's and twenty seven Non Hodgkin's Lymphoma)were examined by B ultrasound before and after chemotherapy.Results There were 86 1% cases with splenomegaly and all cases with hypoechoic nodules in spleen before chemotherapy.According to the ultrasonographic features,it could be divided into three types:nodular,diffuse miliary and massive types.In our study,69 4% cases were nodular type.After chemotherapy,sonography showed that the spleens shrinked and the imfiltrated.nodules of spleens disappeared in most cases.Conclusions Ultrasonography plays a great role in the diagnosis before chemotherapy and in the monitoring after chemotherapy of malignant lymphoma with spleen infiltration.It provides a non invasive and convenient way for the clinical diagnosis,therapy and the evaluation of curative effect.
