宫腔镜电切术治疗子宫黏膜下肌瘤临床观察

【目的】探讨宫腔镜电切术治疗子宫黏膜下肌瘤的疗效及安全性。【方法】回顾性分析在本院行宫腔镜电切治疗的43例子宫黏膜下肌瘤患者的临床资料，并以同期31例行开腹手术治疗者作为对照组。观察两组的手术时间、术中出血量、术后疼痛发生率、肛门排气时间、住院时间和术后并发症情况。【结果】研究组手术时间、肛门排气时间和住院时间均明显短于对照组，差异有统计学意义（P〈0．05）；术后疼痛发生率亦明显低于对照组，差异有统计学意义（P〈0．05）。术后随访6个月，两组均未出现肌瘤复发、宫腔粘连、宫腔感染等并发症。【结论】宫腔镜子宫黏膜下肌瘤电切术较传统开腹手术具有创伤小、手术时间短、术中出血少、术后痛苦小和住院时间短等优点，是目前治疗黏膜下子宫肌瘤的安全有效的方法。

Clinical Observation of Hysteroscopic Resection for the Treatment of Submucosal Myoma of Uterus

Objective]To investigate the efficacy and safety of hysteroscopic resection for the treatment of submucous myoma of uterus. Methods] The clinical data of 43 patients with submucosal myoma of uterus who underwent hysteroscopic resection in our hospital from March 2007 to May 2009 were studied retrospectively. Thirty one patients who underwent open surgery in the same period were selected as controls. The operation time, blood loss, incidence of postoperative pain, anal exhaust time, length of hospital stay and postoperative complications were observed. Results] The operation time, blood loss, anal exhaust time and hospital stay of study group were significantly shorter than those of the control group, and the difference had statistical significance（ P〈0. 05）. The postoperative pain in the study group was also significantly lower than that in the control group, and the difference had statistical significance （ P〈0.05）. There were no complications such as myoma recurrence, intrauterine adhesion and uterine infection in both groups in the postoperative follow-up for 6 months. Conclusion]Hysteroscopic resection of submucosal myoma of uterus is superior to traditional open surgery. It has advantages of lit tle trauma, shorter operation time, less blood loss, postoperative pain and shorter hospital stay, so it is a safe and effective method for the treatment of submucosal myoma of uterus.
