急重型克隆病的诊治分析(附10例报告)

目的：探讨急重型克隆病的临床特点及其诊断和治疗。方法：回顾本院1995～2002年经手术、病理证实的10例急重型克隆病，并结合近期文献进行分析。结果：10例急重型克隆病初诊时全部误漏诊。其中9例行了外科手术治疗，5例行病变肠段切除。10例病人中8例缓解出院，2例住院期间死亡。8例均获随访，其中4例无复发，2例轻度复发，2例已死亡。结论：急重型克隆病误漏诊的主要原因是对本病认识不足，临床上对此病没有高度的警惕性；总结出6点临床特征；病变肠段切除是有效的手术方式，但仍强调内科保守治疗特别是皮质激素治疗的重要性。

THE ANALYSIS OF TREATMENT AND DIAGNOSIS OF ACUTE AND SERIOUS CHROHN DISEASE(APPENDED 10 CASES)

Objective:To investigate the clinical characteristics ,treatment and diagnosis of Acute and Serious Chrohn Disease.Methods:To retrospectively analyse the 10 patients of Acute and Serious Chrohn Disease who were diagnosed by operation and histology from 1995 to 2002 in our hospital, and to analyse them combined with literature.Results:All the 10 patients of Acute and Serious Chrohn Disease were missedly and erroneously diagnosed in the first time, 9 patients were operated on , 5 patients were removed sicked section of intestine, 8 patients have been remittent and discharged, and 2 patients died in the hospital.The 8 out-patients were followed-up, 4 patients were not recurrent, 2 patients were slightly recurrent,and 2 patients were dead.Conclusions:The main reasons of missed and erroneous diagosis of Acute and Serious Chrohn Disease is not knowing this disease fully, and not paying enough attention to this disease in clinical. To conclude 6 clinical characters; removing sicked section of intestine is effective means of operation, but we still emphasize the importance of medical treatment and administration of .Adrenoeorticlal hormones.
