全直肠系膜切除治疗中-低位直肠癌临床疗效分析

目的探讨中-低位直肠癌全直肠系膜切除术对局部复发率和术后生存质量的影响。方法将同期102例中-低位直肠癌患者随机分为TME组（n=51）和非TME组（n=51）。两组均在腹腔镜辅助下完成。TME组采用全直肠系膜切除术,非TME组采用传统术式,比较两组的术后并发症和复发率。结果两组患者一般情况、手术时间比较无统计学意义,P〉0.05。TME组：性功能障碍1例（1.96%）,局部复发2例（3.92%）。非TME组：性功能障碍5例（9.80%）,局部复发10例（19.60%）。两组比较,P〈0.05,均有统计学意义。结论直肠癌采用全直肠系膜切除术,能明显降低局部复发率,不增加并发症的发生,值得临床推广。

Clinical comparison between traditional operation and total mesorectal excision(TME)in the treatment of middle-low set rectal cancer

Objective To explore the life quality and partial recurrence rate of patients after TME treatment of middle-low set rectal cancer.Methods Equally divided 102 middle-low set rectal cancer patients of the corre-sponding period into two groups：TME group and non-TME group.TME group adopts TME operation,while non-TME group adopts traditional operation.Both operations were assisted by a celioscope.The post-operational com-plication and recurrence rates of the two groups were recorded.Results There was no statistical difference in the general situation and time for operation in two groups.There was one patient having sexual dysfunction,two patients having partial recurrence in TME group after the operation,while there were five patients having sexual dysfunction,ten patients having partial recurrence in non-TME group after the operation,and there was significant difference between two groups（P0.05）.Conclusion TME can greatly reduce partial recurrence rate in the treatment of middle-low set rectal cancer,with no chance of complication increasing.
