超声下视觉反馈训练改善全膝关节置换术后功能

摘    要:目的探讨B超引导下股内侧肌的视觉反馈训练对全膝关节置换术后功能改善的效果。 方法采用随机数字表法将昆明市第二人民医院骨科65例全膝关节置换术后患者,分为对照组32例,观察组33例,对照组采取常规护理、健康教育、肌肉力量训练、本体感觉促进训练、关节活动度训练和常规股内侧肌训练,观察组在此基础上,给予B超引导下股内侧肌视觉生物反馈训练,疗程为3周,采用美国特种外科医院膝关节评分(HSS)进行评定。采用独立样本t检验。 结果两组患者在治疗3周后,HSS评分中各因子分及总分较治疗前均有明显变化,观察组各因子及总分均优于对照组,差异有统计学意义(t=6.93,P<0.05)。 结论通过B超引导下股内侧肌视觉生物反馈训练,更有利于全膝置换术后患者运动功能的改善,值得临床推广。

Abstract:ObjectiveTo investigate the effect of visual feedback training of medial femoral muscle guided by B ultrasound on the improvement of knee function after total knee arthroplasty. MethodsSixty-five patients after total knee arthroplasty in the department of orthopedics of the Second People's Hospital of Kunming were selected, and were randomly divided into two groups by random number table method: 32 patients in the control group and 33 patients in the observation group. The control group received routine nursing, health education, muscle strength training, proprioception training, range of motion training and conventional medial femoral muscle training.Besides all these, the observation group were given visual biofeedback training of the medial femoral muscle under the guidance of B ultrasound. The course of treatment was three weeks and was assessed using the Hospital for Special Surgery knee scoring system (HSS). The data were analyzed by t test. ResultsAfter three weeks of treatment, all factors and total scores in HSS scores of the two groups were significantly changed compared with those before treatment. All factors and total scores in the observation group were better than those in the control group, and the difference was statistically significant (t=6.93, P<0.05). ConclusionThe visual biofeedback training of the medial femoral muscle under the guidance of B ultrasound is more conducive to the improvement of the patients' motor function after total knee arthroplasty, which is worthy of clinical promotion.
