临床药师自主设计并应用肠外营养医嘱智能分析工具的实践

摘    要:目的:自主设计肠外营养医嘱智能分析工具,并调研其应用实效。方法:基于微软电子表格软件设计肠外营养医嘱智能分析工具;通过医院信息系统随机抽取肠外营养医嘱两组,由审方药师分别进行人工计算审核和智能计算审核,比较分析其审核速度和准确度;回顾性调研某院2017年1月至10月和2018年1月至10月期间应用肠外营养支持的住院患者病例,点评其肠外营养医嘱合理性,比较分析工具应用前后肠外营养医嘱合格率变化。结果:运用肠外营养医嘱智能分析工具审核肠外营养医嘱的速度和准确度均显著高于人工审核(P<0.01);该工具推广应用后,2017年和2018年同期相比肠外营养医嘱合格率平均由13%提升至80%。结论:肠外营养医嘱智能分析工具的开发应用有助于提高医院审方药师的工作效率和肠外营养医嘱合格率,临床药师在肠外营养临床规范化应用中扮演了重要角色。

Abstract:OBJECTIVE To design a parenteral nutrient prescription-analyzing system and check its validity. METHODS The Parenteral Nutrient Prescription-analyzing System was designed based on Microsoft Excel. two groups of parenteral nutrition prescription were randomly selected through the HIS system, and were manually and intelligently reviewed by the trial pharmacist to compare and analyze their review speed and accuracy.The hospitalized patients who received parenteral nutrition support from January to October 2017 and from January to October 2018 in this hospital were retrospectively investigated, and the rationality of their parenteral nutrition prescription were reviewed to compare and analyze the changes in the qualification rate of parenteral nutrition prescription before and after the application of the tool.RESULTS The speed and accuracy of the application of the intelligent analysis tool of parenteral nutrition order were significantly higher than manual review(P<0.01). After the application of the tool, the rational rate of parenteral nutrient prescription in our hospital increased from 13% to 80% on average in 2018 compared with the same period in 2017.CONCLUSION The development and application of Parenteral Nutrient Prescription-analyzing System could help to improve working efficiency and qualification rate of parenteral nutrient prescription. Clinical pharmacists play an important role in the rational use of parenteral nutrition in our hospital.
