摘    要:目的:建立人血浆中同时测定卡马西平和文拉法辛浓度的HPLC-MS/MS方法,并用于临床癫痫伴抑郁患者血药浓度的测定。方法:色谱柱:Phenomenex Kinetex XB-C18,(2.6 μm,50 mm×3 mm);流动相:甲醇(A)-2%甲酸水溶液(B)(65∶35),等度洗脱;流速为0.4 mL·min-1;柱温:40℃;进样量:5 μL。电喷雾离子源(ESI),多离子反应监测(MRM),正离子扫描。结果:卡马西平在0.1~20 μg·mL-1内线性关系良好,最低定量下限为0.01 μg·mL-1;文拉法辛在10~800 ng·mL-1内线性关系良好,最低定量下限为10 ng·mL-1。准确度在±15%之间,日内、日间精密度的RSD均小于15%,提取回收率为89.24%~105.74%,稳定性良好,不受基质效应影响。结论:本方法简单、快速、经济,具有良好的专属性和灵敏性,适用于临床检测卡马西平和文拉法辛的血药浓度。

Abstract:OBJECTIVE To establish a HPLC-MS/MS method for the simultaneous determination of the concentrations of carbamazepine and venlafaxine in human plasma, and to determine the serum drug concentration of epileptic patients with depression.METHODS Analysis was performed on a Phenomenex Kinetex XB-C18, (2.6 μm, 50 mm×3 mm). Mobile phase:methanol (A)-2% formic acid aqueous solution (B)(65:35), isocratic elution; flow rate:0.4 mL·min-1; column temperature:40℃; injection volume:5 μL. Electrospray Source Ion (ESI), Multiple Reaction Monitoring (MRM), positive ion scanning.RESULTS Carbamazepine had a good linear relationship in the range of 0.1 to 20 μg·mL-1, and the lowest limit of quantification was 0.01 μg·mL-1. Venlafaxine had a good linear relationship in the range of 10 to 800 ng·mL-1, and the lowest limit of quantification was 10 ng·mL-1. The accuracy was ±15%. The RSD of inday-day and day-day precision was less than 15%, and the recovery ranged 89.24%-105.74%. The method has good stability and is not affected by matrix effect.CONCLUSION The method is simple, rapid, economic, specific and sensitive, and suitable for the determination of carbamazepine and venlafaxine in serum.
