联合筋膜鞘悬吊术治疗中重度上睑下垂的效果评价

目的探讨联合筋膜鞘悬吊术治疗中重度上睑下垂的临床疗效及安全性。方法按照随机数字表法将某医院2017年1月—2019年1月收治的中重度上睑下垂单眼患者50例,分为两组,每组25例。对照组行额肌瓣悬吊术治疗,观察组行联合筋膜鞘悬吊术治疗,比较两组上眼睑肌肌力(levator function,LF)、上睑缘角膜映光距离(Marginal Reflex Distance-1,MRD-1)及并发症发生率。结果术后6个月,观察组LF为(6.13±1.29)mm,高于对照组的(4.81±1.21)mm,(t=3.730,P<0.05);观察组术后6个月MRD-1为(4.03±1.31)mm,高于对照组的(2.83±1.23)mm,(t=3.340,P<0.05);观察组并发症发生率与对照组比较,差异无统计学意义(P>0.05)。结论联合筋膜鞘悬吊术治疗可改善中重度上睑下垂患者的LF及MRD-1指标,且未明显增加并发症发生率。

Clinical efficacy and safety evaluation of combined fascia sheath suspension in treatment of moderate and severe blepharoptosis

Objective To investigate the clinical efficacy and safety of combined fascia sheath suspension in the treatment of moderate and severe blepharoptosis. Methods According to the random digital table method, 50 patients with moderate and severe blepharoptosis from January 2017 to January 2019 were divided into a control group and a trial group, with 25 cases in each group. The control group was treated with frontalis muscle flap suspension, and the trial group was treated with combined fascia sheath suspension. The muscle strength of upper eyelid muscle (levator function, LF), the distance of corneal light reflection from upper eyelid margin to (marginal reflex distance-1, MRD-1), and complications were compared between the two groups. Results After the operation, LF in the trial group was significantly higher than that in the control group , and MRD-1 in the trial group was significantly higher than that in the control group . The incidence of complications in the trial group was 8.00%, which was not significantly different from that in the control group (16.00%, P > 0.05). Conclusion The combined fascia sheath suspension can improve LF and MRD-1 in the patients with moderate and severe blepharoptosis, but does not significantly increase the incidence of complications.
