摘    要:目的 探讨连续性孕期营养指导对瘢痕子宫再次妊娠阴道试产成功率及母婴结局的影响。方法 选取2018年3月—2019年3月本院妇产科收治的瘢痕子宫再次妊娠产妇86例,根据随机数表法分为2组,每组43例。对照组按照手册指南进行常规产检,观察组在对照组的基础上对产妇进行连续性孕期营养指导,比较2组产妇阴道试产成功率、阴道分娩产程时间及母婴结局。结果 观察组阴道试产成功率(88.37%)高于对照组(67.44%),剖宫产率(11.63%)低于对照组(32.56%),差异有统计学意义(χ2=5.472,P<0.05);观察组产妇阴道分娩第一产程、第二产程时间分别为(397.43±64.81)min、(46.92±10.56)min,短于对照组的(478.20±89.35)min、(71.78±11.49)min,差异有统计学意义(P<0.05);2组第三产程时间相比,差异无统计学意义(t =1.515,P>0.05);观察组母婴结局较对照组优,差异有统计学意义(P<0.05)。结论 对瘢痕子宫再次妊娠产妇实施连续性孕期营养指导,可有效提高阴道试产成功率,缩短产程时间,改善母婴结局。

Abstract:Objective To investigate the effect of continuous nutrition guidance during pregnancy on the success rate and maternal and infant outcomes of vaginal trial delivery in scar uterus. Methods From March 2018 to March 2019, 86 pregnant women with scar uterus treated in Department of Gynecology and Obstetrics, Zhengzhou Maternal and Child Health Hospital were randomly divided into a control group and a trial group, with 43 cases in each group. The control group was examined according to the manual guide, and the trial group was given the continuous nutrition guidance during the pregnancy on the basis of the control group. The success rate of vaginal trial delivery, the time of vaginal delivery and the outcomes of mother and infant were investigated and compared between the two groups. Results The success rate of the vaginal trial delivery in the trial group was higher than that of the control group (88.37% vs. 67.44%), the rate of cesarean section was significantly lower than that of the control group (11.63% vs. 32.56%), the time of the first stage and the time of the second stage of vaginal delivery in the trial group were (397.43±64.81) min, and (46.92±10.56) min respectively, which were significantly shorter than those in the control group, but there was no significant difference in the time of the third stage of labor between the two groups (t=1.515, P>0.05). The maternal and infant outcomes in the trial group were superior to those in the control group, and the difference was statistically significant (P<0.05). Conclusion The continuous nutrition guidance during pregnancy has a significant effect on increasing the success rate of vaginal trial delivery in scar uterus, shortening the labor process time, and improving the maternal and infant outcomes.
