个性化功能锻炼在微创全髋关节置换术后康复中的应用

摘    要: 目的 探讨个性化功能锻炼在微创全关节置换病人康复中的应用效果。 方法 全髋关节置换手术病人31例,采用随机数字表法分为对照组14例和干预组17例,对照组采用常规康复护理措施,干预组行个性化功能锻炼。比较2组病人术后日常生活自理能力(ADL)评分、髋关节功能(Harris)评分、满意率、术后并发症发生率、住院时间及住院费用以评价护理效果。 结果 干预组术后第1、3、5天的ADL评分均高于对照组相应时间点的ADL评分(P < 0.05~P < 0.01),干预组出院日Harris评分高于对照组(P < 0.01),但2组术后1个月ADL评分、Harris评分差异无统计学意义(P>0.05);2组围手术期并发症发生率差异无统计学意义(P>0.05);干预组病人住院满意度高于对照组(P < 0.05),住院时间和人均住院总费用均低于对照组(P < 0.05)。 结论 个性化功能锻炼应用于微创全髋关节置换,能促进病人术后早期髋关节功能恢复,缩短住院时间、节约医疗资源,提高病人满意度,值得临床推广。

Application value of the individualized functional exercise during recovery after minimally invasive total hip arthroplasty

Abstract: Objective To explore the effects of the individualized functional exercise during recovery in patients treated with minimally invasive total hip arthroplasty. Methods Thirty one patients treated with minimally invasive total hip arthroplasty total hip arthroplasty were randomly divided into the control group(14 cases) and intervention group(17 cases).The control group was nursed with conventional rapid rehabilitation nursing measures, and the intervention group was nursed with individualized functional exercise.The postoperative daily life self-care ability(ADL) score, hip joint function(Harris) score, satisfaction rate, incidence rate of postoperative complications, length of stay and hospitalization expense were compared between two groups, which was used to evaluate the nursing effects. Results The ADL scores in intervention group on the first, third and fifth day after surgery were higher than those in control group at corresponding time points(P < 0.05 to P < 0.01), the Harris score in intervention group on the day of discharge was higher than that in control group(P < 0.01), but there was no statistical significance in ADL score and Harris score between two groups after 1 month of surgery(P>0.05).There was no statistical significance in the incidence rate of perioperative complications between two groups(P>0.05).The hospitalization satisfaction in intervention group was higher than that in control group(P < 0.05), and the length of hospitalization and total cost of hospitalization per capita in intervention group were lower than those in control group(P < 0.05). Conclusions The application of individualized functional exercise in minimally invasive total hip arthroplasty can promote the early recovery of hip function, reduce the length of stay, save medical resource and improve patients' satisfaction, which is worthy of clinical promotion.
